It's Encounter Time! I try to make an encounter using nothing but published material and established rules, and working within a level appropriate XP budget, yet with the intent of earning a TPK as quickly and painlessly as possible. Consider this the DM rebuttal to 4e's "CharOppers."

Today's Encounter is from the Something Awful forum, from a delightful thread just full of these things. I can post weeks of Wednesday Encounters without ever having to do any actual work! This one is a rework of SA forum user ritorix.
Standard Difficult Encounter for 5 level 14 PCs (6800 XP)
2x Bodak Skulk (2800 xp)
16x Lich Claw (4000 xp) (Note: You can adjust the number of Lich Claws up or down to make this qualify as Easy or Hard. It will still be hard either way)
Set Up: If you want to be especially cruel, set it up so the Lich Claws are not on the board at the beginning due to LoS, but can rush around a corner and pin down the players on the first turn.
Tactics: Bum rush the party with Lich Claws to bring Anchoring Claws into effect, and then start throwing out Death's Touch, focusing on the Defenders. Due to the Tiny size of the Claws, you are going to suffer casualties from OAs, but you should still get enough into range for their attacks. Have the Bodaks get into range for their Death Gaze, and start hammering away at anyone Weakened. If you see an opportunity, feel free to bring the Bodaks close enough for Agonizing Gaze to effect the front line troops.
